Marriage Licenses
Appointments are Required to Apply for a Marriage License
Congratulations on your upcoming marriage! Due to limited staffing, it is required that you make an appointment to apply for your marriage license. Please call our office at (920) 849-1458 to schedule.
Application Checklist
- Arrangements must be finalized prior to applying for a license.
- Wedding date
- City, village or township and county where marriage is taking place
- Name, address, phone number, and e‐mail address of officiant
- Schedule an appointment to apply for your marriage license. You can apply through any county. If you wish to apply through our office, please call (920) 849‐1458.
- Certified birth certificate with a raised seal. A hospital certificate of birth or state Notification of Birth Registration is not acceptable.
- Valid photo ID. If a photo ID is not current/valid, proof of residency is required. A recent piece of mail is acceptable.
- Social Security Number
- Documents from your previous marriage. If previously married, provide proof of how last marriage ended (death certificate; legal annulment; or findings of fact, conclusions of law and judgment of divorce).
- $100 application fee. If qualified to apply for a license less than four days prior to the wedding, a $25 waiver fee will be applied. Approval must be given by the County Clerk.
- Parental Consent Form if under 18 years of age.
Marriageable Age
Any person who has attained the age of 18 may marry. Persons, 16‐17 years of age, may marry only with the written consent of parents or guardians. Consent forms must be signed in the County Clerk’s Office.
License Application
Both parties must apply together in person at any County Clerk’s Office within the State of Wisconsin. The application should be made 4‐60 days prior to the marriage date. An appointment is necessary to apply in Calumet County and can be made by calling 920‐849‐1458. The application fee of $100 is payable by cash or check only on the date of the application.
Proof of Age
Both parties are required to provide a certified birth certificate which has a raised seal on it from the State or County where they were born. Hospital souvenir birth certificates and State Notification of Birth Registrations are not acceptable. If born outside the U.S., a Resident Alien Card or passport will be accepted.
Photo ID and Proof of Residency
Both parties must present photo identification (ex: driver’s license, alien card, school ID) and documentation showing a current address (ex: driver’s license, current mail).
Social Security Numbers
Both parties must provide their social security number. It is not necessary for you to bring the card.
Ceremony
When you apply for your marriage license, we will need to know the following information:
- Date of your ceremony
- Location of your ceremony (town, village or city and county)
- Name, address, phone number, and email address of the person performing your ceremony
Divorce Papers/Death Certificates
If either party has been previously married, they just provide a copy of the Divorce Judgment, Legal Annulment Papers or Death Certificate from their LAST marriage. If the last marriage ended in divorce, the divorce must be final for at least 6 months to remarry in Wisconsin.
Foreign Language
Documents in another language must have a certified translation. Persons who do not speak and/or understand English must bring an independent interpreter not related to either party when applying for their license.
Couples wishing to be married at the Calumet County Courthouse should contact the Circuit Court at (920) 849‐1465 Ext.3512 prior to applying for the marriage license. The schedule fills up quickly, so our office recommends calling at least one month in advance.
Wedding ceremonies are performed in Courtroom #2 (Room 115) by appointment only.
Amending a Wisconsin Marriage Certificate
The Wisconsin Office of Vital Records Office determines if a court order is required to amend a Wisconsin marriage record.
The Calumet County Clerk of Courts cannot commence an action regarding amending a Wisconsin record unless the petitioning party received an amendment packet from the Wisconsin Vital Records Office.
How to correct a marriage record when the marriage occurred less than 365 days ago
If the marriage occurred less than one year ago, the officiant or the county clerk who issued the license can make corrections. For more information, please contact the vital records office.
How to correct a marriage record when the marriage occurred more than 365 days ago
If the marriage occurred more than one year ago, a court order is needed to make corrections.
If you have a certified copy of the record:
Mail it to the vital records office. With it, add a note explaining what is incorrect and what the correct information should be. They will look over the record and send you the court order form and instructions if needed.

If you don’t have a certified copy of the record:
Apply for a certified copy using the normal application process. Submit your application explaining what is incorrect and what the correct information should be. They will look over the record and send you the court order form and instructions if needed.
Forms/Documents Required to Commence an Action
- Form F-05093– Court Order to Amend a Wisconsin Marriage Certificate
- This document is included in the amendment packet from the Wisconsin Office of Vital Records. This form is not available at the Clerk of Courts Office.
- Petitioning party is to complete the pertinent sections.
- This form cannot contain erasures, cross-outs, write-overs, correction tape or correction fluid.
- Form CV-487 Petition to Amend Wisconsin Vital Record
- Current certified copy of the marriage license.
- Any evidence which verifies the current marriage certificate is incorrect.
Filing Instructions
- Once all the necessary forms are completed, the petitioning party may take them along with the current certified copy of the marriage certificate, statement, and evidence to the Calumet County Clerk of Courts to have them filed.
- A Filing fee of $164.50 must be paid
- You must check with the Clerk of Courts Office to see how payment is accepted (Cash, checks, money orders, debit or credit cards).
- A court case number and circuit court judge are assigned to the case
- The Clerk of Courts Office will forward all documents and supporting evidence to a Circuit Court Judge for review
- If the Petition is approved, the Court will sign the Court Order to Amend a Wisconsin Marriage Certificate.
- If the Petition is not approved, the Court may schedule a court hearing.
Recording an Amended Marriage Certificate
Upon the court official granting the request to amend the marriage certificate and signing the Court Order to Amend a Wisconsin Marriage Certificate, the Calumet County Clerk of Courts office will apply a seal to the document. The Clerk of Courts Office will mail the Court Order to Amend a Wisconsin Marriage Certificate to the Wisconsin Office of Vital Records with the petitioning party’s check/money order to cover the fees and for any additional copies of the amended marriage certificate.
Vital Records Fees:
- $10.00 vital records fee
- $20.00 for certified copy of the amended marriage certificate
- $3.00 for each additional copy of the amended marriage certificate issued at the same time as the first
- Checks/money orders are to be made payable to State of Wisconsin Vital Records. Cash, debit or credit cards are not accepted for vital records fees.
- An authenticated copy of the Court Order to Amend a Wisconsin Marriage Certificate, Petition, and supporting evidence will be returned to the petitioning party by the Calumet County Clerk of Courts via first class mail.
- It may take 3-4 weeks to receive your requested copy(s) of the amended marriage certificate from the Wisconsin Office of Vital Records.
