Based on recent IRS guidance, Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) will no longer be automatically sent to employees. Your Form 1095-C is available upon request by contacting Human Resources via email at HRDept@calumetcounty.org, by telephone at (920) 849-1611, or by mailing your request to Calumet County, Attn: Human Resources, 206 Court St., Chilton, WI 53014. Upon receipt of your request, a copy of your Form 1095-C will be provided within 30 days.
